STANDAARD DOSERING (15-60 min voor incisie)
- cefazoline iv 2000mg*, na 4 uur gift herhalen : cefazoline iv 1000mg†
- clindamycine iv 600mg, herhalen bij operatieduur > 6 uur
- vancomycine iv 1000mg#, herhalen bij operatieduur > 12 uur
* bij wondsluiting binnen 1 uur na toedining cefazoline iv kan worden volstaan met 1000mg
† bij resterende operatieduur > 3 uur cefazoline iv 2000mg
# starten tot 120 minuten voor de ingreep gezien de benodigde inlooptijd
AANGEPASTE DOSERING OBESITAS
- cefazoline iv 3000mg (BMI > 40), na 4 uur gift herhalen : cefazoline iv 2000mg*
- clindamycine iv 900mg (gewicht > 180kg), herhalen bij operatieduur > 6 uur
- vancomycine iv 10mg/kg (gewicht > 100kg), herhalen bij operatieduur > 12 uur
* bij resterende operatieduur > 3 uur cefazoline iv 3000mg
